Performance Reviews
It is the requirement of the University of Washington to conduct formal performance evaluations at least annually for Contract Classified, Classified Non Union, and Professional Staff employees.
The recommended process is as follows:

Give a blank form to each employee to complete a self-evaluation

The supervisor also completes an evaluation of the employee

Then the two of you meet to compare and discuss evaluations

Both the employee and supervisor need to sign and date the final evaluation and return it to me

The employee gets a copy (I will be glad to take care of that, or you can do it yourself just let me know which you prefer)

If the employee disagrees with your assessment of the evaluation, s/he can attach comments to the final evaluation; both will be kept in the personnel file
Evaluations are required annually. The performance evaluations need to be completed by June 30th.

Professional staff have two options:

During the evaluation session between the employee and the immediate supervisor, performance factors and goals for the next review period. Job descriptions should be updated to reflect current and/or new responsibilities.

Every year to comply with UW School of Medicine policy, each employee is to complete and sign a new Privacy, Confidentiality and Info Security Agreement . Please return the signed completed forms to HR.

The following items should be routed to HR for retention for three years in employee file: performance evaluation, updated position description information (if applicable), and the Info Security Agreement.
In conjunction with the annual performance review process, supervisors should prompt employees to take the following required trainings:

(REQUIRED ANNUALLY) Asbestos Training Annual Asbestos Awareness Certification & Training

Please forward the confirmation to the gsadmin [ a t ] uw.edu so we can track this data centrally. We must receive a copy of the email confirmation or the certificate for employees to receive credit for completing the course.

(REQUIRED EVERY 3 YEARS) Biosafety Training

(REQUIRED EVERY 2 YEARS) Violence Prevention Training
NOTE: Performance evaluations shall not be used to initiate personnel actions such as transfer, promotion or discipline; however, evaluations may serve as supporting documentation for personnel actions. Notwithstanding the current state legislation salary freeze, annual performance evaluations remain an important practice and are required annually under UW policy.
